Our Claim team will be normally ask for the following documents, upon receiving the notification :

1. Chronology of the events – including date of Participant first made aware if the incident/potential claim;
2. Treatment/surgery performed on the patient (claimant, a minor);
3. Details of Doctor attending to the said patient and whether they are independent consultants/in house doctors – for hospital;
4. Any further details/information that the Participant could provide in relation to this claim.
